|Last Friday, Peter Bisbecos, Director of DDRS along with Ellen McClimans, Director, BQIS sent out Health & Safety Notices per issues with consumers under Indiana's Medicaid Waiver Program. These issues were discovered through Indiana's Mortality Review Committe who reviews deaths of consumers. Below is an email I submitted back to Mitch Roob & Peter Bisbecos that shows the lunacy of the state right now per our waiver program. the critical point is, the state has reduced funding of adequate services for all consumers. Limited staff are available in the homes to ensure the health, safety & welfare of consumers (which is a federal Medicaid law under the waiver) due to funding cuts. This is the only way providers can sustain under the rate cuts; reduce staffing in the homes.
But now, IFSSA/DDRS is so confused and don't know which way to turn in this mess, they are focusing energy and our tax paying dollars on insane issues like consumers having weiners and peanut butter! Will the Oscar Mayer weiner-mobile show up during BQIS surveys? Is Mr. Peanut going to be the next Director of Peanut Butter monitoring? What is going on?
Below is my email to Peter Bisbecos & Mitch Roob:
To All It May Concern:
I want to take direct quote from the Peter Bisbecos bulletin of 9/15/05 per the Annual Plan project. "The intent of this effort is to create an environment in which we can all spend more time with individuals we serve and less time on paperwork." This was a very nice "concept" back to the beginning of the Annual Plan project, but this effort has gone very awry.
On 5/25/06, all providers received the Health & Safety Notices written by Peter Bisbecos & BQIS based on concerns identified by the Mortality Review Committee. In this panic mode, DDRS has now submitted more "unpromulgated" rules per these thirteen Health & Safety areas which outline a multitude of additional "paperwork" that now must be completed. And in the "micro" management style DDRS has decided to take, the biggest laugh is over the Dining Risk directive concerning consumers having weiners and peanut butter!!!!
I must quote this laughable directive/policy; "For individuals with ANY problems chewing, swallowing or choking, peanut butter and hot dogs should be removed from the diet and all staff trained to understand why this is important. For individuals who have no known problems with chewing, swallowing or choking, the team should address the risk of including hot dogs and peanut butter in the individual's diet. IF the team decides to have these foods available to the individual, the foods should be used with extreme caution and staff should have training specific to the dangers." Does anyone realize how ridiculous this policy is??? Is this all DDRS has to do in this time of devastation of Indiana's Medicaid Waiver program. And since when is the Medicaid Waiver program designed to micro-manage consumers in the Home & Community Based Services program? Whatever happened to freedom of choice, dignity of risk, learning to integrate within the community and live like the rest of us?
I find these latest policies to be a joke but more important, a huge disregard to your own words per the workings of your Annual Plan project. Less paperwork......not in this regard. And if you are now moving the Annual Plan project to be a "micro" management system where every move, food and cough has to be outlined in an ISP, procedure, tracking sheet and training class, you better be increasing those rates for all providers. As the saying goes, "if you want filet mignon type service, you can't be paying for the price of ground beef." If you want services to be perfect and without any risk, you have to pay the cost for those perfect services.
