This information was included with the listing of Medicaid Waiver contacts, but we thought it important enough to pull out and bring to your attention.

The Developmental Disability Waiver Ombudsman receives, investigates and attempts to resolve complaints regarding Medicaid Waivers. You should include a call to the Ombudsman in addition to your appeals and legislative contacts.

Thanks for the name, Nancy. With that we were able to find this from a paper from FSSA entitled, "WHO'S WHO RESOURCES IN THE WORLD OF AUTISM AND DD WAIVERS," dated October, 2002 :

Ombudsman for Persons with Developmental Disabilities....1-800-622-4484 or (317) 234-1181 (within the Indianapolis calling area).

The statewide DD Waiver Ombudsman Program applies to individuals who have a developmental disabilitydisability and who receive services under a Medicaid Waiver under the Federal Home & Community Based Services Program. By law, the ombudsman shall receive, investigate, and attempt to resolve complaints and concerns that are made by or on behalf of an individual who is developmentally disabled and who receives services under a Medicaid Waiver. The ombudsman shall report results of any investigation to the complainant. If no investigation is done, the complainant shall be notified of the decision not to investigate and the reasons for the decision. Contact the DD Ombudsman, Brian Reynolds, at one of the numbers above or at BReynolds@fssa.state.in.us. The DD Ombudsman is a staff member of the Bureau of Quality Improvement Services.
